What to expect when scheduling your appointment

  1. After scheduling an appointment, an intake form will automatically be sent to you. Please complete AND submit the form.
  2. Provide medical documentation of your certifying condition. You may upload your documentation or sign our Medical Records Request form so that we can request records.
