• You do not currently have a medical card.
• You have a diagnosis that will qualify (see below).
• You have access to medical documentation that verifies your condition.

1. After booking your appointment, you will receive an intake form by email. Complete and submit the form PRIOR to your appointment.
2. You will need to upload your medical documentation (preferred) or sign our Medical Records Request form so we can request them from the medical office that has documented your diagnosis.
3. On the day of your appointment, you will meet with Dr. Charlie Booras, MD, followed by a meeting with our medical dosing educator, Peter Booras.
4. If you meet the criteria for certification, we will submit this information to the Florida State (OMMU) right after your appointment. The OMMU will then send you an email to register for your Medical Card at a cost of $75 per year (paid to the state).
5. You will typically receive email notification of your approval ON THE SAME DAY. You may use this approval to go shopping for product at the dispensary of your choice.
