Schedule your cannabis evaluation appointment online with BoorasMD. Once confirmed, you will then be emailed an electronic questionnaire form. Fill out and e-sign intake form. Be sure to SUBMIT the completed form.

You should be able to get records containing your diagnosis from your health care provider. Many people have access to their records using a Patient Portal

You can bring these with you to be scanned or simply give us a copy.

You may also upload documentation/medical records using IntakeQ (it is secure & HIPAA compliant).

You can request that records be faxed to us from the health care professional who diagnosed your condition.

You will be seen by Dr. Charlie Booras, MD, a Certified Cannabis Physician, for the evaluation of medical cannabis treatment.

You will also have a personalized consultation with Peter Booras, our Certified Cannabis Educator during your visit.

Education. Knowledge is power and we want you to be fully informed of your cannabis treatment, including the Florida laws, types of cannabis for your specific needs, and dosage recommendations.

Guidance on where to obtain your cannabis medicine and what to look out for. Your cannabis care will be tailored to fit your specific medical needs.

Follow-up is also very important. We encourage all our clients to keep in touch with us so that we make adjustments to the treatment plan, if need be.

Once certified by Dr. Booras, our Cannabis Educator will enter you in to the Medical Marijuana Use Registry. In order to receive your legal medical cannabis ID card, you will need to submit a copy of your photo State ID/proof of residency and pay the state-required application fee of $75.00.
